This in-depth case study set out to illuminate advantages and constraints of one community based health insurance scheme, the Salamba Women's Health Insurance Scheme. The study was carried out in Northern Ghana in July 2003 using qualitative research methodologies such as focus group discussions, participatory rural approach and in-depth interviews with key informants.

- to give recommendations for future improvements in the health service in Fielmuo Subdistrict and other rural health institutions

The reason for the low utilization of FP in Wellembelle is multifactorial. It is clear from the above, that the perception of FP differs between sex and age groups. To address the problem of low utilization of FP it is therefore crucial to understand the different sex and age groups in order to approach each group with the right focuses.

The communities have a consistent knowledge about the purpose of the HC. It is however striking that two out of three groups of young women do not mention supervised deliveries as a purpose of the HC. There is a profound language barrier witch makes it very difficult for the midwife to educate the mothers. Often the patient contact is very limited because of this. For the same reason the feedback given on the ANC examinations is also insufficient.As mentioned the men believe that both ANC and deliveries at the HC is very expensive. Since they are the main decision-makers it is very important that they get a true knowledge on the costs of the services.
